Finasteride (Propecia) for Hair Loss: Benefits and Side Effects

Finasteride, marketed as Propecia, is a prescription medication that works by inhibiting 5-alpha reductase, the enzyme that converts testosterone to DHT. Since DHT is the primary driver of androgenetic alopecia, blocking its production can halt hair loss and promote regrowth.

In my practice, finasteride has been transformative for many patients. Men who start finasteride early often experience stabilization of hair loss within 3-6 months and noticeable regrowth within 12 months.

The most important thing to understand about finasteride is that it works systemically—it affects DHT levels throughout your body, not just on your scalp. This is why it's so effective, but it's also why potential side effects must be discussed.

The most commonly reported side effect is sexual dysfunction, though this occurs in only 1-2% of users. Most men tolerate finasteride very well. Other potential side effects include decreased libido and changes in ejaculate volume.

These side effects are usually reversible—they typically resolve within a few months of stopping the medication. However, it's important to discuss these potential risks with your doctor before starting treatment.

Finasteride is not recommended for women of childbearing age because it can affect fetal development. Post-menopausal women can sometimes use it off-label, but this requires careful consideration.

One critical point: finasteride only works while you're taking it. If you stop, hair loss typically resumes within 6-12 months. This is why I emphasize that hair loss treatment is often long-term.

My recommendation: if you're a man with androgenetic alopecia, discuss finasteride with your dermatologist. For most men, the benefits far outweigh the risks. Combining finasteride with minoxidil produces even better results.
